You are completing this survey because you live within the area served by the North Lanark Community Health Centre located in the village of Lanark. We are funded by the Champlain Local Health Integration Network to provide primary health care services and health promotion programs to individuals, families and the community of north Lanark. We have been working to address the health needs of our community since 1993. As part of our recent planning process, we consulted community partners and leaders, our volunteers, Board and staff to help us identify strategic priorities for our organization for the coming years.
